3 - a - State the total salaries and allowances paid during the last month to employees who receive monthly salaries in
the shop or establishment.
b - State the total wages paid during the last month to workers (on a daily wage) in the shop or establishment.
c - State the cost of free services provided to employees or workers during the last month (if both exist)
and these services include food, clothing, housing, or health services... etc.
d - State the amount of rent for the shop or building paid during the last month, and if the rent is annual, it is divided by the number of months
of the year.
e - If the building belongs to the owner of the shop or establishment, state the estimated monthly rent for it.
f - State the cost of building maintenance incurred during the year and divide it by 12 months. This maintenance includes painting the building and minor repairs
and does not include additions to the building such as building a new room... etc. Added to that is the cost of transportation fees paid by the shop or
establishment to complete its work, as well as the cost of printing, stationery, telephone, water, and electricity paid during the last month.
g - If the shop is a hotel, restaurant, or private hospital, state the value of food and groceries purchased for customers and guests during

i - State the total of other expenses paid during the last month which were not mentioned above, if any.
4 - State the total amount paid during the last month by the owner of the shop or establishment to his household to cover all living expenses including
rent, food, clothing, transportation expenses, water and electricity fees, educational expenses, or any other expenses spent by the shop owner
or establishment on his household during the last month, and state in the following field the total of all those expenses which the owner of the
