* If the number of persons to be mentioned exceeds (26), please use three more copies of this form to mention the additional persons, provided that the word (Continued) is placed on them and the words (To be continued) on the first three copies, and they are bundled together when sent to the Liwa Education Directorate.
* In the field "Nominal salary with allowances", the total nominal salary with all allowances, such as cost-of-living allowances and lecture fees, shall be written, provided that it is free of any deduction, including pension deductions, estimated in Dinars and from his school only. If your teacher works in another school, his salary is divided proportionally to the number of classes he teaches in both schools, and the amount he receives should be appropriate for his work in your school only.
* In the field "Teachers' classes not less than the quota", write the total weekly classes taught by the teacher, provided that he has a full quota or a full quota with lectures. If he teaches less than his quota even by one class per week, the number of his classes must be mentioned in the field "Weekly classes for those with a partial quota". The principal and assistant principal are considered to have a partial quota even if they teach their quota or more.

First - The Principal is considered to have a partial quota, and the total of his weekly classes taught in the school is written, whether they are a full quota, partial, or more than his quota.
Second - The Assistant Principal is considered to have a partial quota, and the total of his weekly classes taught in the school is written, whether they are a full quota, partial, or more than his quota.
Third - Every teacher who teaches less than his quota even by one class per week, whether on the permanent staff, seconded, an employee, or a lecturer.
* In the field "Job Title", write: Principal, Assistant Principal, full-quota teacher in the school, partial-quota teacher in the school, administrative employee (which includes head of clerks, clerks, accountants, etc.), others (which includes all those not mentioned such as janitors, binders, guards, etc.).
* In the field "Nationality", write (Iraqi), (Arab) if he is a citizen of an Arab country, (Foreigner) for non-Iraqis or non-Arabs.
* "Qualification": Qualifications are classified as follows, and it is sufficient to include the numerical sequence in the qualification field to indicate the category of the qualification:-
1 - A teacher seconded for teaching in secondary or vocational schools or teachers' training institutes.
2 - Diploma from higher institutes (general vocational training and not prepared for the teaching profession).
3 - Diploma from higher institutes (prepared for the teaching profession).
4 - Diploma from higher institutes (general training).
5 - Bachelor's degree or its equivalent (from colleges with training for the teaching profession).
6 - Bachelor's degree or its equivalent (from colleges with general training).
7 - Master's.
8 - Other qualifications.
The meaning of the numerical sequence is that if the teacher holds a Bachelor's degree from the College of Education, his qualification category is symbolized by the number (5) only, and so on for other qualifications.
* In the field "Subjects taught", write the lessons taught by the teacher. As for the Principal and Assistant who do not teach any subject, (Exempt) is written in front of their names in this field.
* In the field "Specialization", write the specialization of the Principal, Assistant, teacher, or instructor in one of the subjects listed below:-
1 - Religion.
2 - Arabic Language.
3 - English Language.
4 - Mathematics, including (Arithmetic, General Mathematics, Algebra, Trigonometry, Geometry, Solid Geometry).
5 - Natural Sciences, including (General Science, Chemistry, Physics, Zoology, Botany, Biology, Health).
6 - Social Studies, including (History, Geography, Civics, Economics, Sociology and Philosophy, Society and its Problems).
7 - Home Economics, including (Home Management, Housework, Sewing, etc.).
8 - Physical Education.
9 - Fine Arts.
10- Practical subjects, including (Commerce, Blacksmithing, Gardening, etc.).
11- Other specializations.
* This form must be filled with the required information as soon as you receive it and without delay, and it must be returned to the Liwa Education Directorate after filling it with information within a maximum period of 11/9/1967.
* The accuracy of the information you recorded in the form must be verified before sending the first and second copies to the Liwa Education Directorate.

Please read the form and its instructions with complete accuracy before starting to fill it. In case of any ambiguity or
confusion in understanding the contents of the form, please refer to the Directorate of Education to assist you. We ask the school
administrations to exert special care when filling out the forms and including correct information because incorrect
information will not help in the educational planning process in addition to confusing the work of the Directorate. Knowing
that educational planning depends fundamentally on correct and accurate statistical information. The ⟦line⟧

Further to our letter No. 58881 dated 11/23/1967, we list below some clarifications related to
statistical form No. (8). Please note the following points:
1- It includes all workers in the school regardless of their job. Conversely, any teacher or employee who does not work in
the school, such as being assigned to work in the Directorate or another school, their name is not included in the aforementioned form.
2- If the school includes a kindergarten and elementary level and is managed by one principal, the form should be sent to us
containing information for both the kindergarten and elementary together. Not to send two forms, one for the elementary
and another for the kindergarten.
3- The nominal salary is written merged with the cost of living allowance without any deduction of any kind, and it is taken from the payroll
list.
4- If the teacher works in more than one school, such as completing their quota in another school, their salary is divided according to
the proportion of their work in each school proportionately with the number of classes they perform. Consequently, each school writes its
respective information about them, not that the full salary is written in their original school and their salary is written in the second school's form
resulting in repetition.
5- If classes in the school are distributed according to the weekly class schedule and the teacher's classes are less than the quota,
the number of classes is listed in the field for those with a full quota, except for reserves; the word "reserve" is marked in the field for those with an incomplete quota next to their name.
6- What is meant by age in years is to write, for example, 21 - 22 - 34 - 50 and so on, and not the date of birth.
7- If the teacher holds two or more degrees, only the numerical sequence representing the highest degree is written.
8- Evening elementary schools are also responsible for entering information, and the name of the teacher on the permanent
staff is recorded as well as the lecturer's name; in the salary field, the average of their lecture fees is written in Dinars.
9- The school principal must deliver the form to the Statistics Division personally to identify deficiencies and complete them
in their presence, or deliver it to the competent employee to ensure it is complete to avoid sending it incomplete and returning it again
which causes delay and damage to forms, and consequently wasting the effort exerted. Thank you.

Further to our letter No. 58881 dated 23 / 11 / 1967, we record for you some clarifications regarding form
Statistics No. (9). Please note the following points /
1 - It includes all employees in the school regardless of their work; conversely, every teacher, administrative employee, or servant
who does not work in the school, such as being assigned to any other entity, his name is not listed in the mentioned form.
2 - Age refers to the number of years the person has lived, for example 34 - 48 - 51 - and so on, and not the year
of birth. Similarly for the length of service, it is written as 3 - 5 - 14 - 25, and those recently appointed (this year)
their service is considered one year.
3 - The nominal salary is written merged with the cost of living allowance without any deduction of any kind, and this is taken from the payroll
list.
4 - If lessons were distributed according to the schedule and the teacher's lessons are less than the quota, the number of lessons is listed in the field
for those with full quotas, except for substitutes, where the word "substitute" is marked in the field for those with incomplete quotas in front of his name.
5 - Private day and evening secondary schools are also responsible for entering information, and the teacher's name on
the permanent staff is recorded as well as the lecturer's name. In the salary field, the average amount received for lecture fees is written, estimated

to be filled together by placing carbon paper between one copy and another. The first and second copies are sent to the Directorate of Education of
the Province and the third copy is kept in the school records.
Please read the form and its instructions with complete accuracy before starting to fill it. In case of any ambiguity or
confusion in understanding the contents of the form, please refer to the Directorate of Education to assist you. We ask school
administrations to take special care when filling out the forms and including the correct information because
incorrect information will not help in the educational planning process in addition to disrupting the work of the Directorate. Note
that educational planning depends primarily on correct and accurate statistical information. The
