* This form is to be filled in triplicate simultaneously by placing carbon paper between copies. The first and second copies are sent to the District Education Directorate, and the
third copy is kept in the school records.
* In the field "Number of Students in the School", write the total number of students present in the school and attending it at the time of filling out the form, distributed by grade and gender.

Notes:-
First: Write in the job title field - Principal, Assistant Principal, Teacher, Contract Teacher, Assigned Teacher, Chief Observer, Observer, Assistant Observer, Clerk, Accountant, Assistant Accountant, Employee.
Second: Write in the nationality field - Iraqi, Arab if a citizen of an Arab country, Foreign for non-Iraqis and non-Arabs.
Third: In the staff field - write (Permanent) if the teacher is on the permanent staff of the school. (Assigned) if assigned to work in the school from another school; as for a teacher assigned from your school to work in another school, they are not included within
the staff of your school.
As for the teacher assigned from your school to work in another school, they are not included within your school's staff. (Lecturer) if the teacher delivers lectures in your school.
(Quota Lectures) for a teacher who delivers lectures in your school to complete their quota.
Fourth: Degree - Degrees are classified as follows, and it is sufficient to include the numerical sequence in the degree field to indicate the category of the degree:
1 - Higher Institute Diploma (prepared for the teaching profession).
2 - Higher Institute Diploma (general preparation).
3 - Bachelor's degree or its equivalent (from colleges with preparation for the teaching profession).
4 - Bachelor's degree or its equivalent (general preparation).
5 - Master's.
6 - Teachers' Houses or its equivalent (assigned) to work in secondary schools.
The purpose of mentioning the numerical sequence is, if the teacher holds a Bachelor's degree from the College of Education, their degree category is symbolized by the number (3) only, and so on for other degrees.
Fifth: In the subjects field, list the names of the subjects taught by the teacher; underline their specialized subject if the teacher teaches more than one subject.
Sixth: Specialization means the teacher's specialization in one of the subjects listed below:-
1 - Religion.
2 - Arabic Language.
3 - English Language.
4 - Mathematics, including Arithmetic, General Mathematics, Algebra, Trigonometry, Geometry, Solid Geometry.
5 - Natural Sciences, including General Science, Chemistry, Physics, Zoology, Botany, Biology, and Health.
6 - Social Studies, including History, Geography, National Duties, Economics, Sociology.
Seventh: The Principal and Assistant Principal also have their degrees listed, and they are considered to have a full quota in terms of the number of weekly lessons.
7 - Arts, including Drawing, Sculpture, Acting, Music.
8 - Home Management, including Home Economics, Household Tasks, Sewing, Knitting, Embroidery, Decoration.

Form No. (8) Number of Teachers Employed in Kindergartens and Primary Schools
Instructions: -
* This form shall be filled out in three copies at once by placing carbon paper between one copy and another. The first and second copies are sent to the Liwa Directorate of Education, and the third copy is kept in the school records.
* If the number of persons to be mentioned in this form is more than (26), please use three copies of this form to mention the additional persons therein, provided that the word (Continued) is placed on them and on the first three copies the word (To be continued) and when
sending them to the Liwa Directorate of Education, they shall be bundled together.
* In the field "Nominal salary with allowances", the total nominal salary with all allowances, such as cost of living allowances and lecture fees, shall be written, provided that it is free of any deduction, including pension deductions, estimated in Dinars and from
this school only, and if your teacher completes his quota in another school, his salary is divided proportionally to the number of classes he teaches in the two schools, and what he receives appropriate to his work in your school only is mentioned.
* In the field "Teacher classes that are not less than the quota", the total weekly classes taught by the teacher shall be written, provided that he is of full quota or full quota with lectures. If he teaches less than his quota even by one class
per week, the number of his classes must be mentioned in the field "Weekly classes for those with a partial quota". The principal and assistant principal are considered to have a partial quota even if they teach their quota or more.
* In the field "Weekly classes exceeding the quota and taught as lectures in this school", the total classes taught by the full-quota teacher as lectures in this school shall be written.
* In the field "Weekly classes taught in another school", the total weekly classes taught by the principal, assistant, and teacher in any other school, whether official or private, shall be written.
* In the field "Weekly classes for those with a partial quota", the total weekly classes for every teacher who teaches less than his legal quota even by one class shall be written. This expression includes the persons whose titles are listed below: -
First. The principal is considered to have a partial quota, and the total of his weekly classes taught in the school is written, whether his quota is full, partial, or more than his quota.
Second. The assistant principal is considered to have a partial quota, and the total of his weekly classes taught in the school is written, whether his quota is full, partial, or more than his quota.
Third. Every teacher who teaches less than his quota in your school.
* Write in the job title field (Principal, Assistant Principal, Full-quota teacher in the school, Partial-quota teacher in the school, Others including employees such as janitors and guards .. etc.
* Write in the "Nationality" field (Iraqi) (Arab) if he is a subject of one of the Arab countries, (Foreigner) for non-Iraqis or non-Arabs.
* Qualifications: Qualifications are classified as follows, and it is sufficient to include the numerical sequence in the qualification field to indicate the category of the qualification: - This includes the principal, the assistant, and every teacher teaching in your school.

15- Vocational secondary (Home arts).

* This form is specifically for primary school pupils. If the school has kindergarten classes, please use form No. (1) to show
their numbers in addition to this form.
* In the field "Number of pupils in the school," the total number of those present in the school and attending it at the time of filling the form is written, and those who left the school are included. The number of failing pupils
is within the number of existing pupils and not separate from them, according to their grades and sections.
* In the field "Number of repeating pupils," the total number of pupils who failed in their class last year and continued studying in the same class this year is written. Thus,
the number of repeating pupils is extracted from the total number of pupils in the school in the field "Number of pupils in the school" and detailed separately in this field.
* A "Boys" school is a school where all its pupils are males only.
* A "Girls" school is a school where all its pupils are females only.
* A "Mixed" school is a school that contains males and females at the same time, even if it is one boy with several girls or one girl with several boys.

Visits to the facilities of the Baghdad General Water Liquefaction Authority are limited to the fifth primary grades
