Instructions -
1 - Every student wishing to enter the university shall fill out the four cards found
inside the envelope with complete accuracy and precision.
2 - The school stamp shall be placed on the photograph and the grades shall be certified by the school principal
and the Provincial Director of Education as indicated on the cards.
3 - The four cards, placed inside the envelope, shall be handed to the student himself, and he or his
representative shall submit them to the Registration Department at the University of Baghdad - Al-Waziriyah after the
submission dates are announced on the Iraqi Radio and in local newspapers.
4 - The matter of student admission will be considered in light of the information shown on the cards, and after
the university announces the names of those accepted into colleges and institutes, each of them shall submit the following
documents to the college or institute where they were accepted.
A - School certificate
B - Civil status book and a certified copy of the Iraqi nationality certificate or the nationality certificate of the country
to which the student belongs.
C - Certificate of non-conviction
D - Smallpox vaccination certificate
E - Any other document requested by the relevant college
6 - Please emphasize that a student is not entitled to acquire more than one set of cards
(four cards in four colors placed inside the university envelope) because their number is very limited,
and we suggest organizing a list containing the name of every student to whom the cards were delivered.

Further to our letter No. 46408 dated 22 / 7 / 1959.
1 - It has been decided to organize the new preparatory school certificate with numerical sequence
in the Provincial Directorate of Education, and after completing its organization, it is sent to the relevant schools to stamp the photo
and have it signed by the school principal.
2 - The certificate must be free of any erasure or linguistic error
3 - The name shall be recorded in the certificate exactly as it is recorded in the general examination results.
4 - The additional photo you send to the certificate register must be identical to the photo pasted on
the certificate.
5 - Old certificates are sent to the Central Education Store for cancellation.
6 - The same serial number listed in the certificate shall be recorded in the certificate register, in addition to the date
of its registration.
7 - The photo is pasted on the certificate in the upper left side opposite the Republic's emblem fixed on the right
side.
8 - The stamp is placed directly under the photo and cancelled with the school seal.
